Harrison Morton Spring Sport Physicals
3/8/2022 11am-3pm
What to bring: Section 1-6 must be filled out by athlete and parent and turned in to the Athletic Directors Office by 3/4/2022, Section 7 will be filled out on day of physical by staff and physicians 3/8/2022. Section 8 Recertification, must be completed ONLY if athlete participated in a Fall/Winter sport.
